Pemberian anti nyeri golongan Anti Inflamasi Non Steroid (AINS) pada nyeri sendi sering dilakukan, namun efek samping pemberian menimbulkan permasalahan tersendiri di bidang kesehatan. Propolis suatu bahan alami telah banyak dikenal dan dikonsumsi sebagai makanan tambahan dan penghilang nyeri sendi lutut, mengandung bioflavonoid dan Caffeic Acid Polyphenol Ester (CAPE), suatu scavenger yang berpotensi sebagai anti inflamasi dan anti nyeri. Synovitis merupakan proses awal peradangan sendi, ditandai dengan meningkatnya jumlah makrofag pada synovium yang berperan penting terhadap kerusakan kartilago dan tulang melalui pembentukan fibroblast. Beberapa penelitian membuktikan efek propolis sebagai anti inflamasi, namun mekanisme kerjanya dalam menekan jumlah makrofag dibandingkan dengan anti inflamasi lain belum pernah diteliti. Penelitian ini mempelajari efek anti inflamasi propolis dibandingkan dengan celecoxib terhadap jumlah makrofag pada synovitis lutut tikus yang diinduksi peptidoglikan melalui pengamatan spesifik makrofag dengan CD-68. Penelitian bersifat semikuantitatif laboratorium eksperimental, menggunakan tikus jantan galur wistar yang dibagi: Grup 1 hanya diberi peptidoglikan intra artikular 100ug dosis tunggal; Grup 2 diberi peptidoglikan intra artikular 100ug dosis tunggal dan propolis peroral 100mg/kgBB/hari; dan Grup 3 diberi peptidoglikan intra artikular 100ug dosis tunggal dan celecoxib peroral 50 mg/kgBB/hari. Data penelitian adalah jumlah skor dari jumlah makrofag dan synovitis sendi lutut tikus yang diamati secara mikroskopis melalui imunohistokimia CD-68 dan pewarnaan HE pada hari ke-1, ke-3 dan ke-14. Perbedaan skor masing-masing grup dianalisa secara statistika dengan ANOVA. Peningkatan jumlah makrofag dan synovitis untuk semua grup penelitian terjadi pada hari ke-3, selanjutnya menurun pada hari ke-14. Terdapat perbedaan penghambatan jumlah makrofag antara grup 1 dengan kedua grup lainnya pada hari ke-3 secara bermakna (p<0.05). Hal tersebut membuktikan bahwa terdapat peran anti inflamasi dalam menghambat jumlah makrofag. Pada hari ke-3 dan ke-14, penghambatan terhadap jumlah makrofag grup 2 lebih besar dibandingkan dengan grup 3 dengan perbedaan mencapai 500 % (p<0.05). Propolis menghambat jumlah makrofag pada hari ke-3 dan 4-5 kali lebih kuat dibandingkan dengan celecoxib. Synovitis yang terjadi akibat induksi peptidoglikan menunjukkan derajat ringan sampai sedang berdasarkan skor Krenn. Administering Non Steroid Anti Inflammation Drug (NSAID) in pain management of arthritis treatment still has a health problem regarding its complications. It occurs due to long time treatment and adverse effect of NSAID. Propolis, a natural bee product, is recognized as food supplement and traditional pain killer at knee joint pain. It contains a natural scavenger named Flavonoid and Caffeic Acid Phenolic Esters (CAPE) that potent to inhibit inflammation process. Several studies are performed to elucidate a role of Propolis to eliminate pain in joint inflammation, but its effect is still unclear. Hyperplasia and hypertrophy of synovial following synovitis which occur due to either inflammation (in rheumatoid arthritis) or non inflammation (osteoarthritis) will stimulate proliferation of the synovial resident macrophage. The number of macrophage, cytokines production and fibroblast activity which function to destruct the cartilage and bone will be increased. The study will clarify the differences of anti inflammation effect between Propolis and Celecoxib in the synovitis process of the rat knee toward increasing of macrophage cell numbers which evaluated by immune expression CD-68. Method of the study is experimental laboratory using Wistar rat. They are decided into 3 group; 1: Peptidoglycan only, 2: Peptidoglycan and Propolis, and 3: Peptidoglycan combined with Celecoxib. Each group will be sacrificed in D-1, D-3 and D-14. The sample of study is a histopathology change of the rat knee joint tissue which is identified by HE staining and immunohistochemistry of CD-68 microscopically. It is scoring based on severity of joint destruction and macrophage-CD 68 labeled cell numbers. The differences of macrophage cell number scoring within the study groups are analyzed by ANOVA. The severity degree of synovitis occurs at D-1, peak at D-3 and decreased at D-14 similar with previous study. The effect of proliferation inhibiting of macrophage is different between Group 1 and another group significantly. Peptidoglycan-induced synovitis will stimulate activation and proliferation of macrophage in D-3 (p<0.05). Either administering Propolis or Celecoxib will inhibit proliferation of macrophage, dominantly in Propolis treatment, but statistically the differences effect between Propolis and Celecoxib are not significant (p>0,05). It could be concluded that the peak of Propolis effect occurred in D-3 and shows 4-5 folds stronger than Celecoxib in D-14. The severity grade of synovitis-Peptidoglycan induced occurs in mild until moderate based on Krenn’s score.
