Toxic Epidermal Necrolysis Pada Penderita Lupus Nefritis Yang Diterapi Dengan Metilprednisolon Dosis Denyut

Latar Belakang : Angka kejadian drug eruption meningkat pada penderita lupus eritematosus sistemik (SLE). SLE dilaporkan pula sebagai etiologi yang memungkinkan dalam epidermal necrolysis (TEN). Kasus : Dilaporkan satu kasus wanita berusia 36 tahun penderita SLE yang mengalami TEN setelah minum obat karbamazepin. Gambaran klinis berupa makula dan papula eritema, vesikel, palpable purpura, serta erosi pada wajah, leher, dan punggung dengan epidermolisis 35% disertai proteinuria +++. Terapi yang diberikan yaitu metilprednisolon intravena dosis denyut 1000mg selama 3 hari untuk menangani lupus nefritis, dikombinasi dengan siklofosfamid intravena dosis denyut 1000mg setiap bulan selama 6 bulan dan metilprednisolon oral serta terapi topikal berupa kompres NaCl 0,9%, krim desoksimetason 0,25% dan krim tabir surya. Perbaikan klinis pada TEN didapatkan pada hari ke-4 dan perbaikan pada lupus nefritis didapatkan pada hari ke -9 (proteinuria -). Diskusi : Penderita SLE mempunyai risiko tinggi untuk terjadinya TEN. Penanganan utama pada TEN yaitu menghentikan obat yang diduga sebagai penyebab. Pada penderita ini tidak diberikan terapi sistemik khusus untuk TEN karena penderita telah diterapi kortikosteroid sistemik dan siklofosfamid dosis denyut untuk lupus nefritis yang dapat pula menghentikan TEN, serta memberikan hasil yang baik.

Background : Systemic lupus erythematosus (SLE) patients have a higher incidence of experiencing drug eruption. SLE was also reported as a possible ethiology of toxic epidermal necrolysis (TEN). Case : A case of TEN due to carbamazepine in a 36-year-old woman with SLE was reported. The clinical manifestations were erythematous macules and papules, vesicles, palpable purpura, and erosions on the face, neck, and back with epidermolysis of 35% of the body surface area and proteinuria +++. The patient was treated with pulse dose 1000mg intravenous methylprednisolone for 3 days to treat the lupus nephritis, combined with pulse dose of 1000mg intravenous cyclophosphamide every month for 6 months, also oral methylprednisolone, and topical therapy of 0,9% NaCl wet dressing , 0,25% desoximethasone cream and sun screen lotion. Improvement of TEN was achieved the 4th day of therapy and the lupus nephritis was improved of the 9th day of therapy (proteinuria-). Discussion : SLE appears to confer a higher risk of the development of TEN. The main management of TEN was to stop consuming drugs that were suspected as the causative agent. This patient was not given any spesific therapy for TEN because she was already treated with pulsed dose of systemic corticosteroids and cyclophosphamide for the lupus nephritis which also could halt the progression of TEN, and gave good results.
