Background: Coledochal cyst (CC) is a rare congenital dilatation of the bile ducts. The estimated incidence in Western countries varies between 1 in 100,000 and 1 in 150,000. The incidence is higher in Asia and occurs more common in females, with a male to female ratio of 1:3 to 4. The treatment and early diagnosis of CC are very important to prevent lethal complications of the disease. The etiology of choledochal cysts is still unclear. The incidence of malignancy increases with age, supposed to be 0.7% below 10 years of age to 14.3% after 20 years of age. Objective: To report a case of 4-year-old girl diagnosed with choledocal cyst Case: AT, a 4-year-old girl, admitted to Dr. Hasan Sadikin Hospital with jaundice as chief complaint since 1 month before admission, with right upper abdominal pain and mass. Physical examination findings revealed jaundice and liver enlargement. Laboratory findings: ALT 125 U/L, AST 116 U/L, alkaline phosphatase 1,154 U/L, and gamma GT 1,083 U/L and HBsAg and antiHAV lgM non-reactive. The abdomen sonography showed signs of choledocal cyst. The patient will be operated. Conclusion: The appropriate management for choledocal cyst is surgery and must be performed as soon as possible to reduce the risk of malignancy
