Background : Meckel’s diverticulum is a common asymptomatic congenital gastrointestinal anomaly, but it might occur only with vomitus and constipation. The rarity and non-specific symptoms make the diagnosis uncertain. The incidence of Meckel’s diverticulum was 1% to 2% of the population. Objective : To report an unusual manifestation and diagnosis of Meckel’s diverticulum in children. Case : A Z 2-year-old boy who presented at pediatric emergency ward with vomitus, after having treated at a primary health center and two different hospitals without any improvement. His vomiting became worse and got severe dehydration. Case (cont’d): Laboratory findings revealed: slightly anemia, slightly hypmatremia, slightly hypocalcemia During treatment, patient showed weight gain and well rehydrated, but with abdomen distension and constipation. Ultrasonography revealed donuts sign as a sign of obstruction. The patient was consulted to pediatric surgeon to undergo a surgery. Case (cont’d): During the abdominal exploration, inverted Merkel’s diverticulum was diagnosed and resected. Successfid diagnosis and treatment of this disease requires a high index of clinical suspicion, which is becoming increasingly relevant to general pediatrician. At present, the patient is clinically stable. Conclusion: The physician ought to be suspicious of this condition since the non-specific symptoms and the rarity of it make diagnosis uncertain. Early surgical intervention may ensure a favorable outcome
